About
Every year, community-based organizations (CBOs) save the healthcare system significant amounts of money by addressing social care gaps for community members and patients. Unfortunately, current data infrastructure and funding mechanisms do not provide a clear path for CBOs to quantify their impact or to receive appropriate compensation for reinvestment in their organizations and communities.

Value-based social care (VBSC) challenges the status quo and helps CBOs get paid for the social services that they already provide. The VBSC model improves community health, data-driven innovation and provides a bridge to sustainable partnerships.

In this workshop, CBOs will learn about:
- Value-based payment models in healthcare contracting
- How VBSC can benefit CBOs
- How CBOs can access new and sustainable funding streams through VBSC
- VBSC implementation
- Joining a VBSC network

Who should attend:
Community based organizations that provide direct and essential services to community members, including, but not limited to:
- Housing
- Food
- Caregiving Support
- Home Visiting
- Case Management
- Affordable Childcare
- Transportation
Organizations and individuals interested in learning about value-based social care and innovative approaches to social determinants of health.

We look forward to a wonderful conversation about VBSC with CBO leaders in the Chicago area!
