Advance care planning (ACP) is the term used to describe the process of discussing and planning ahead between a patient, their family and the professional about the patient’s future wishes and priorities for care, in anticipation of some deterioration in their condition. ACP offers patient’s the opportunity to plan their future care and support, including medical treatment, while they have the capacity to do so. Highlighted as one of the most difficult areas for professionals, advance care planning can greatly help guide patients, families and professionals in decision-making for care at the end of life. This study session will aim to explore the importance of ACP and will give you information, advice and support on how to approach ACP discussions with patients and explore some of the documentation used to record and share these discussions.
