The emPOLST system is a state-of-the-art, web-based system developed and managed by NJHA. It securely houses electronic POLST forms and provides the ability for licensed physicians and advanced practice nurses to create a POLST form and provide patients the ability to access their forms in “read only” mode. It also offers patients the ability to make their POLST form accessible to their clinical and non-clinical care teams.

Ms. Edelstein is Senior Vice President of NJHA’s Center for Partnerships Transforming Health (PaTH). She directs the policy, regulatory, education and membership services activities of the Association as they relate to post-acute and long term care. In addition, Theresa coordinates policy work related to insurance and managed care and works directly with NJHA’s emergency preparedness team. Earlier in her career, Theresa worked as a licensed nursing home administrator in New Jersey.

Theresa has her Masters in Public Health from the Columbia University, Mailman School of Public Health and her Bachelor of Science from Georgetown University. She has presented and published nationally and internationally on topics related to post-acute care services, quality and policy.
